Our aim at Clare Road Medical Centre is to provide a full general practice service including consultations with a doctor or nurse. As well as confidential GP consultations we provide health screens, childhood immunisations, blood tests, referrals to specialists and home visits if you are unable to attend the clinic. COVID-19
Flu Vaccination 2021/22
All flu vaccination this year will require an appointment. We will be unable to accommodate walk-in patients for this purpose and priority will be given to the most vulnerable patients in the first instance.
Please note the flu vaccine will not protect against COVID-19

VISITING CLARE ROAD MEDICAL CENTRE
The health and safety of our patients and staff is paramount at Clare Road Medical Centre.
We kindly request that all patients and visitors to the surgery arrive wearing face mask or face covering.
We are unable to provide face mask at the surgery.
Failure to this request may result in individuals being denied entry into the building.
All patients are reminded that we are not able to provide walk-in appointments. We are offering telephone appointment in the first instance.

Repeat prescription
Repeat prescription service is available. Prescriptions requests will only be taken over the telephone during this time and via the online services.
All prescriptions will be sent to a Pharmacy of your choice, who will collect them from the Surgery.
Alternatively, prescriptions can be collected from surgery by patient.
Prescription Collction time: 12.00-13.00 or 16.00-17.00
Practice require a minimum of 48 hours (business working days) prior to collecting your repeat prescription.

Please note that the first 7 days of absence are covered by the Self-Certification Scheme
f you are unable to work for longer than 7 days or you are on long term sick and your existing certificate is running out, please telephone the Surgery.
Online self isolation notes launched - providing proof of coronavirus absence from work
People unable to work for more than seven days because of coronavirus (COVID-19) can obtain an isolation note through a new online service.
SELF ISOLATION CERTIFICATES can be accessed through the NHS website 111.nhs.uk/isolation-note
During this challenging time telephone lines are extremely busy, please avoid ringing unless it is absolutely necessary.

Stay at home if you have Coronavirus symptoms
Stay at home if you have either:
The latest advice is to self-isolate for 14 days if you have a new continuous cough, high temperature or loss of taste or smell.
If you live with other people, the entire household should stay at home for 14 days from the day the first person gets symptoms.
If you live with someone who is 70 or over, has long-term condition, is pregnant or has a weakened immune system, try to find somewhere else from them to stay for 14 days, If you have to stay at home together, try to keep away from each other as much as possible.

You should only call 111 if the symptoms checker advises you to.
Please do not attend you GP Surgery, Community Pharmacy or A&E department if you have any symptoms of Novel Coronavisrus to stop the spread of infection.
You can help stop the spread of Novel Coronavirus by ensuring you practise good hygiene. Cough and Sneeze into a tissue, bin it and immediately wash your hand with soap and hot water for at least 20 second.

Chronic disease refers to medical conditions that may have an ongoing impact on your health. In particular we use this term to refer to the following conditions:
Hypertension, Diabetes, Asthma, COPD, CKD (Chronic kidney disease),
Mental Health conditions such as schizophrenia or bipolar depression,
Ischaemic heart disease, Heart Failure, Stroke, mini-stroke (TIA), Rheumatoid , Arthritis, Osteoporosis, Thyroid disease, Atrial Fibrillation,
Peripheral arterial disease
We encourage our patients to take an active role in helping to manage their own conditions, but it is helpful to come for a review once a year.
We would like to ask for your help in this matter. By taking responsibility for your medical condition you can help ensure that you are having the best possible care.
This review process is important and we will not be able to continue prescribing repeat medications without it.
